肝内胆管癌(iCCA)是一种源自胆管上皮细胞的恶性肿瘤,近年来的全球发病率呈持续上升趋势,具有侵袭性强、进展迅速的特点。外周神经侵犯(PNI)是其重要的病理特征之一,与肿瘤复发、淋巴结转移及患者生存期缩短密切相关。PNI不仅是肿瘤细胞沿神经路径扩散的关键机制,更是肿瘤重塑微环境的主动过程,涉及肿瘤细胞与神经细胞、施旺细胞及其他微环境成分之间复杂的相互作用。本文综述了iCCA中PNI的诊断分级体系、临床意义及机制研究进展,旨在为伴有PNI的iCCA早期诊断及未来治疗策略的制定提供新的见解。

Intrahepatic cholangiocarcinoma (iCCA) is a malignant tumor originating from bile duct epithelial cells, with an increasing global incidence and characterized by high invasiveness and rapid progression. Perineural invasion (PNI) is one of its important pathological features, closely associated with tumor recurrence, lymph node metastasis, and shortened survival. PNI is not only a key mechanism by which tumor cells spread along neural pathways, but also an active process of tumor microenvironment remodeling, involving complex interactions between tumor cells, nerve cells, Schwann cells, and other components of the microenvironment. This article reviews the diagnostic grading system, clinical significance, and mechanistic research progress of PNI in iCCA, aiming to provide new insights for the early diagnosis of iCCA with PNI and the development of future therapeutic strategies.
