目的

通过检测火器伤后白细胞介素6（interleukin-6,IL-6）在血浆和组织中的表达，探讨创伤时IL-6的变化规律及其与创伤严重程度的关系。

方法

制作火器伤模型，24只小型猪随机分为对照组（C组，不致伤）、颅骨切线伤组（H组）、双侧股骨骨折组（L组）和多发伤组（M组），每组6只。采用7TD1（IL-6依赖的杂交瘤细胞株）生物法测定外周血和门静脉血IL-6活性。PCR方法测定各组动物肺、肝组织IL-6 mRNA的表达情况。

结果

C组各项指标无明显变化；各创伤组伤后6 h开始体循环血浆IL-6明显增高。创伤后门静脉血浆IL-6水平变化升高更明显，肝、肺组织IL-6 mRNA的表达明显高于对照组，M组伤后血中IL-6水平和肺、肝组织IL-6 mRNA的表达又较H、L两组更高。

结论

创伤后IL-6在血中和组织中的表达总体上是升高的，表达水平的高低与创伤的程度是一致的。

Objective

To discuss the mechanism of IL-6 changes after firearm injuries and the relationship between the changes and the severity of injuries.

Methods

5.8 mm bullets were used to make various injuries. 24 health pigs were divided into 4 groups. Control group (Group C, n=6) was subjected to catherization only，group H(n=6) was subjected to tangental fracture of parietal bone. Comminuted fractures group (Group L, n=6) was subjected to tangental fracture of parietal bone and comminuted fractures of bilateral femora. Multiple trauma group (group M, n=6) was subjected to the injuries of Group H + L .The bioativities of IL-6 in portal blood and systemic circulation were detected with biological methods. The expression of IL-6 mRNA was assayed in lungs and livers before and after trauma.

Results

The plasma concentration of IL-6 in systemic circulation was increased obviously in 6 hours after trauma. The bioativities in portal blood increased more significantly than that in systemic circulation.The enhanced expression of IL-6 mRNA in lungs and livers were found after trauma. No significant changes were found in group C.

Conclusion

The expression of IL-6 in circulation or tissue are increased obviously after firearm injuries.The concentration of IL-6 may be associated with the injury severity.
