直肠灌洗和直肠癌前切除术后局部复发

背景

脱落的直肠腺癌活性细胞具有种植能力,术中直肠灌洗能减少这些活性细胞的数量和活性,但尚无关于直肠灌洗和直肠癌术后局部复发率关系的结论性的证据。

方法

分析人群登记库中1995~2002年行直肠癌前切除术且随访5年患者的资料。直肠灌洗均由外科医生慎重执行。全国直肠癌患者有97%在统计范围内且随访率达98%。

结果

分析4677例患者资料(3749人接受直肠灌洗,851人未行,77人失访);灌洗组52.0%患者和未灌洗组41.4%患者接受术前放疗(P<0.001)。局部复发率分别是6.0%和10.2%(P<0.001)。一元和多元Logistic回归分析结果支持直肠灌洗:OR值分别是0.56(95%可信区间0.43到0.72)和0.61%(95%可信区间0.46到0.80)(P均<0.001)。对接受外科根治手术患者多元分析OR值是0.59(95%可信区间0.44到0.78;P<0.001)。

结论

直肠灌洗者预后较未直肠灌洗者预后好。
