目的

探讨微小RNA-214(miR-214)和miR-181c在胃癌组织中的表达水平及对预后的影响。

方法

选取2014年1月至2015年1月于川北医学院附属医院收治的68例胃癌患者为研究对象,均接受手术治疗,出院后随访1~60个月。利用实时荧光定量PCR技术检测患者癌组织和癌旁组织miR-214、miR-181c相对表达量;利用Kaplan-Meier曲线进行生存分析;Cox多因素回归分析影响胃癌患者预后的独立危险因素。

结果

胃癌组织中miR-214、miR-181c表达水平均明显低于癌旁组织,差异有统计学意义(P<0.05)。根据miR-214、miR-181c表达均值将患者分为高表达组和低表达组,miR-214、miR-181c表达水平与年龄、性别、淋巴结是否转移无关,与TNM分期、肿瘤分化程度有关(P<0.05)。患者总生存率为44.12%,miR-214低表达组和高表达组术后5年累积生存率分别为35.71%、57.69%,两组间比较差异有统计学意义(P=0.035);miR-181c低表达组和高表达组术后5年累积生存率分别为35.55%、60.87%,差异有统计学意义(P=0.024)。Cox多因素回归分析结果显示,TNM分期高(HR=1.569,95% CI:1.029~2.391,P=0.036)、miR-214低表达(HR=1.643,95% CI:1.294~2.087,P<0.001)及miR-181c低表达(HR=1.327,95% CI:1.045~1.685,P=0.021)是影响胃癌患者预后的独立危险因素。

结论

miR-214、miR-181c在胃癌组织中表达显著下调,与胃癌患者临床病理参数及不良预后有关,参与胃癌的发生发展过程。

Objective

To investigate the expression levels of microRNA-214 (miR-214) and miR-181cin gastric cancer tissues and their influences on prognosis.

Methods

Sixty-eight patients with gastric cancer admitted to North Sichuan Medical College from January 2014 to January 2015 were selected as the study objects, all of them received surgical treatment, and were followed up for 1-60 months. The expression levels of miR-214 and miR-181c in cancer and adjacent tissues were detected by real-time fluorescence quantitative PCR. Kaplan-Meier method was used to analyze the survival curve. Cox multivariate regression analysis was used to analyze the independent risk factors influencing the prognosis of gastric cancer patients.

Results

The expression levels of miR-214 and miR-181c in gastric cancer were significantly lower than those in adjacent tissues, and the differences were statistically significant (P<0.05). According to the average expression levels of miR-214 and miR-181c in gastric cancer patients, they were divided into high expression groups and low expression groups, the expression levels of miR-214 and miR-181c were not related to age, gender, lymph node metastasis or not, but related to TNM stage and tumor differentiation (P<0.05). The overall survival rate of 68 patients with gastric cancer was 44.12%, and the 5-year accumulate survival rate of miR-214 low expression group and high expression group were 35.71% and 57.69%, respectively, the difference between the two groups was statistically significant (P=0.035); the 5-year accumulate survival rate of miR-181c low expression group and high expression group were 35.55% and 60.87%, respectively, with significant difference between the two groups (P=0.024). Cox regression analysis showed that high TNM stage (HR=1.569, 95% CI: 1.029-2.391, P=0.036), low expressions of miR-214 (HR=1.643, 95% CI: 1.294-2.087, P<0.001) and miR-181c (HR=1.327, 95% CI: 1.045-1.685, P=0.021) were independent risk factors for the prognosis of gastric cancer.

Conclusions

The expressions of miR-214 and miR-181c in gastric cancer tissues are down-regulated, which are related to the clinicopathological parameters and poor prognosis of gastric cancer patients, and is involved in the occurrence and development of gastric cancer.
