目的

探讨Hedgehog(Hh)信号通路转录因子Gli2在肝癌中的表达及其与预后的关系。

方法

选取68例肝癌及20例肝血管瘤旁肝组织切除标本,采用免疫组织化学法检测Gli2蛋白表达,并分析Gli2表达与肝癌临床病理特征及预后的关系。

结果

Gli2蛋白在肝癌组织中的表达率(63.2%)高于癌旁组织(11.8%)及正常肝组织(10.0%)(χ2=38.431、17528,均P<0.01);Gli2表达水平与肝癌分化程度、包膜是否完整、有无血管侵犯、早期复发及肝内转移相关(P<0.05);Gli2蛋白高表达患者术后总生存率和无瘤生存率明显低于低表达者(P<0.05);多因素分析表明,Gli2蛋白表达水平是影响肝癌患者术后总生存率及无瘤生存率的独立预后因素(HR=2.239,P<0.05;HR=1.917,P<0.05)。

结论

Gli2的高表达与肝癌的侵袭转移特性相关,可作为评价肝癌患者术后预后的标志物之一。

Objective

To explore the expression of transcription factor Gli2 of Hedgehog (Hh) signaling pathway in human hepatocellular carcinoma (HCC) and its correlation with prognosis.

Methods

Immunohistochemistry was used for detection of Gli2 protein in sixty-eight pairs of HCC tissues and 20 samples of normal liver tissue from patients with liver hemangiomas. The correlation between Gli2 expression and clinicopathologic characteristics and prognosis were analyzed.

Results

The expression of Gli2 in HCC tissues (63.2%) was significantly higher than in paraneoplastic liver tissues (11.8%) and normal liver tissue (10.0%) (χ2=38.431、17528, all P<0.01). The expression level of Gli2 was significantly correlated with tumor differentiation, encapsulation, vascular invasion, early recurrence, and intrahepatic metastasis (P<0.05). The overall survival (OS) and disease free survival (DFS) rate of the patients with high Gli2 expression was lower than that of the patients with low Gli2 expression (P<0.05). The Cox multivariate analysis demonstrated that among the factors analyzed, Gli2 expression was an independent prognostic factor for OS and DFS in HCC patients after a curative resection (HR=2.239, P<0.05; HR=1.917, P<0.05).

Conclusion

The abnormal expression of Gli2 is closely related to invasion and metastasis of HCC and it can be used as a marker to evaluate the postoperative prognosis for patients with HCC.
